Collaborating locally to improve services
Dr Ben Saw is the lead GP in Neighbourhoods for children and families. He has worked in Hackney for 13 years, previously working as a Clinical Director of a local PCN (Primary Care Network).
Dr Saw is always looking to make new connections with local health practitioners and community groups to improve his knowledge of what support is available to local people and ensure local GPs are able to work together with local partners to provide high quality care.
There is now a directory for schools and GPs to safely share information, and a children’s social prescribing pilot has begun to save GPs time and provide a Community Navigator to support young people to reach the services they need.
On an individual level, it can be challenging for different services to keep each other up to date. Being part of MDM’s ensures Dr Saw and other services come together on a regular basis to discuss the support they’re providing, and deliver the best care for that person.
