An update on the Neighbourhoods Programme from Dr Aimee Henderson
The aim of the City and Hackney Neighbourhoods Programme is to improve the lives of local people by bringing together health and care services in each Neighbourhood closer to the people who live there, who understand what is needed best. Over the Autumn a lot of work has been happening to create the right structures and support for this new way of working.
Homerton Health Care have been recruiting new care coordinators to encourage people with moderate frailty and long-term conditions to take up activities and services that will help them stay well for longer. Managers working on this ‘anticipatory care’ programme are also looking for people with lived experience to meet up and work with them to design a local name for ‘anticipatory care’ that is meaningful to residents and to help problem solve as the new service gets going in the different Neighbourhoods. City and Hackney partners know the importance of supporting the rich stream of non-medical care professionals both in the voluntary and health sectors that can really support people to get to services and activities that can increase activity, social life and connection and support financial wellbeing. A new 3 year strategy has been co-produced with people in these roles such as social prescribers, care coordinators and wellbeing coaches. The aim is to make sure that this important sector feels valued and connected into the wider health and care workforce of the 8 Neighbourhoods. This includes network meetings where any roles that navigate health and care for people in some form can discuss their work, build relationships and focus on the key issues of each Neighbourhood. We are particularly keen that smaller organisations that are better placed to bring services together with sections of our communities that are experiencing greater health inequalities, are able to access training and information about projects that are happening. We have been working on a pilot to support the Neighbourhoods workforce across all the organisations delivering health and care. This includes taking an anti-racist approach to service design, inclusive recruitment to ensure that the lived experience of City and Hackney residents is valued in our recruitment methods, Neighbourhood staff meetings, an introduction to your Neighbourhood visit through the eyes of volunteers and much more. The Neighbourhood Forums where local priorities are discussed between residents, the voluntary sector and NHS and council health and care services have now completed 2 rounds. In each area they are working out what key issues to focus on and are starting to produce work groups taking on the key issues such as the cost of living. The Forums also started the process of deciding how to develop an enabling environment for preventative working with a devolved budget for each neighbourhood. The Forums will be experimenting with hybrid meetings in the next round where people can attend online and in person. Children’s, Young People, Maternity and Families are developing their new Family Hubs to be connected to the 8 Neighbourhoods which brings the opportunity to embed a Think Family mindset into adult health and care services and also to address some of the gaps in transition services for young people.
