Many residents are involved with a range of services, organisations and professionals in different ways to meet different needs.

Multidisciplinary Meetings (MDMs)
Across City and Hackney those residents (who may be living with complexities in their life) will have involvement with and from a range of services, organisations and professionals. All working with them in different ways, to meet potentially different needs. The Neighbourhood Multi-Disciplinary Meetings (MDMs) bring together the people supporting a resident, their families, carers, or those they are close to on a regular basis. This enables a holistic view of the individual to be formed, shared and centred around the outcomes they would like to work towards. The MDMs are a forum for people to access additional support to address wider determinants such as housing or debt, building awareness of how residents can be best supported and empowered.
Each Neighbourhood has a monthly MDM, facilitating the development of partnership working and relationship building between teams, services and people. Some teams have successfully transitioned to work in teams within a Neighbourhood footprint, for example Adult Community Nursing with many other teams working towards this. Neighbourhood working aims to reduce working in organisational silos, by making it easier for people to work together. We are building on our experience of developing MDMs in our pilot on Anticipatory Care.
